Help Me Connect [www.helpmeconnectmn.org] serves as Minnesota’s Part C Resource Directory. Help Me Connect helps expectant families, families with young children, and professionals working with families find and connect to services in their local communities.
Many families who are seeking a referral for developmental concerns through Help Me Grow MN may also want connections for additional family supports that can be found in Help Me Connect. Help Me Grow MN and Help Me Connect work in partnership to help children and families access needed services.
Yes! Service coordinators, case managers, community navigators and others working with young families can order free outreach materials and download translations, videos/podcasts and other resources online. They can also create an account to keep “favorites” lists for each family they are supporting.
Help Me Connect is state funded through MN Statute 145.988 and is a joint initiative between Minnesota's Departments of Children, Youth and Families; Education; Health; Human Services; and the Governor’s Children’s Cabinet.

If you have concerns about a child's development and think a child might need extra help to learn and grow, don't hesitate to refer a child.
The family will be contacted by the local school district to arrange for a screening or evaluation to determine if their child is eligible for Infant and Toddler Intervention or Preschool Special Education services in Minnesota.
